What documentation needs to be submitted in the application for the Mentor Coaching hours?

In your application, you will need to enter your Mentor’s name, email address, date of first session, date of last session, how many hours were completed and if any of those hours were in group. 

You will not need to provide any certificates or documentation for the mentor coaching requirement.
